If you are referring a patient to OSVS and have performed diagnostics at your clinic, then please send the medical record, including radiographs and lab work, to: reception@osvs.net. You may also fax the record or send copies with the client. Our referral form is available for online submission or PDF download for your convenience:

If one of your patients is seen by the emergency service, or by one of our specialists, you will receive an email notification from Shareville. Shareville is our online medical records portal where you can access your patients’ records in real time. You will be notified by email when your patient checks in and checks out of the hospital. The notification emails will include a link to access the Shareville portal. Here is a PDF that includes instructions for how to access Shareville or click here to login. You may login to the portal by creating a password associated with your email address. In the portal, you will have access to download all of your patients’ medical records. After the patient is discharged, the completed medical record should be accessible through the Shareville portal, which will include the discharge and follow-up instructions. Please contact our Referral Coordinator, Kim DiGangi, with any questions at ext. 173.

If you would prefer to have your patient transferred back to your clinic for further care, please feel free to call and speak with the doctor on the case. We also encourage you to call at any time during this patient’s hospitalization if you are interested in being updated on the patient’s condition or would like to be involved in treatment decisions.
